Yes, the comparison was made on already diseased people that take Ivermectin for an extended period of time (annually for 10 to 15 years) There's no info regarding sperm problems for short term Ivermectin users, much less people who are typically healthy, not diseased and not malnourished. We already new that Ivermectin was a drug that's been around a long time to treat a lot of people for a lot of different things. This is just one very small example. There are others.
What we don't know is, if users of the new Covid vaccines today will see their penis fall off sometime in the next 3 to 10 years. :D

If they were malnourished and diseased wouldn’t that hurt their sperm count/motility/dysfunction just the same before and after treatment with ivermectin? Why did it get so much worse after taking ivermectin?
