I think McCain did both. 1) he wanted to thwart Trump, and 2) he had a genuine concern there was no ready-to-go replacement for obummercare. That is what he said. He could have/would have voted to end obummercare if there was a plan to replace it.
But, I also believe it was very personal for McCain. Like Krooked Killary, he thought he was entitled to the WH. It was his turn, by Gawd! And he was insanely jealous of Trump achieving what he could not. And that smallness forever casts a shadow on him and his political career.
